Shingles Survey

HOW TO ENTER

The survey begins at 10AM ET on May 1, 2018. Limit of one online entry per person per day at www.vaccines411.ca. Online entries must be received by 3:00PM (EST), May 31, 2018 (“Survey Closing Date”). An electronic copy is created by filling out the survey form and subscribing to the Vaccines411.ca newsletter.

PRIZE DETAIL

$100 total in prizes: (4) Mastercard gift cards worth $25. The prize must be accepted as awarded and is not transferable or convertible to cash.

THE DRAW

A random draw from all eligible entries received on or before the Survey Closing Date will be made on June 4, 2018 at the FR@SNM office in Dollard-des-Ormeaux, Quebec; all in accordance with the procedure established in the Survey Rules.

Odds of winning depend on the numbers of eligible entries received. A representative of FR@SNM will notify the selected entrant by e-mail within ten (10) days of the draw date listed above. If the selected entrants cannot be contacted within fourteen (14) days, or do not meet all of the survey conditions, they forfeit any rights to the prize, and another entrant will be randomly selected from the remaining eligible entries.

GENERAL RULES

The survey is conducted by FR@SNM and is open to all persons residing in Canada who have reached the age of majority in their province or territory. Employees of FR@SNM, its affiliates, subsidiaries, advertising and promotion agencies, and members of their immediate families or persons whom they are domiciled with are not eligible to enter the survey. All decisions of the jury are final.

By entering the survey and/or accepting the prize, entrants consent to the use of their name in any publicity or advertisement carried out by FR@SNM and on the list of winners without further compensation. Prize is non-transferable, not redeemable for cash and must be accepted as awarded.

The Winners by acceptance of the prizes agree to release FR@SNM and their respective affiliates, subsidiaries, advertising, promotional agencies, promotional partner, and fulfillment house (the “Releases”) from any and all liability claims or actions of any kind whatsoever.

FR@SNM and its agents are not responsible for technical, computer, printing, distribution, or production errors and may terminate or withdraw any survey found to contain such errors without liability and without notice, at their sole discretion. FR@SNM reserves the right to reject any entry form that is incomplete, illegible or that contains a human or mechanical error and to proceed with another entry form. FR@SNM will not be responsible for the failure or malfunction of any computer software, hardware or telecommunications equipment affecting participation in or any other aspect of this survey.

In the event of any discrepancies between the English language rules and the French language rules, the English rules shall prevail.
